CGHS Polyclinic is located in the 3rd floor of Ganesh Towers, Infantry Road, Shivajinagar, Bengaluru. Phone: 080-25589225.The working hours are from 7:30 AM to 2 PM on all working days except Sundays and notified Central Government holidays. You can reach here by bus by alighting at Shivaji terminus or at M G Road Metro Station in case of Metro.
We have the services of the following specialists at Polyclinic: Medical Specialist, Eye Specialist, ENT Specialist, Skin Specialist & Child Specialist along with attached Dental Unit and Laboratory.
Written reference from Wellness Centre is not a must and all beneficiaries irrespective of pay are eligible for direct consultation with Specialists, as per latest GOs. However, it is advisable for them to first consult Medical Officer at the Wellness Centre, who acts as their family physician.
Registration is done at the reception counter and tokens are issued on ''first come, first served'' basis from 7.30 AM onwards. Because of time constraints, number of tokens issued to each specialty is limited. Medicines advised by the Specialists have to be obtained from their respective Wellness Centres. Beneficiaries are required to consult the doctors at their Wellness Centre for needful in this regard. For investigations (advised by Specialists) to be done in lab or CGHS empanelled centre, references are issued by the concerned Specialist. Beneficiaries are requested to accommodate very sick or the very elderly in availing the services, on priority.
Beneficiaries can take online appointment for Specialists 15 days in advance. They have to report at the reception and register manually at the counter(for all Specialists).The allotted time is only the reporting time and not the actual time of consultation. Beneficiaries coming from distant places are requested to confirm the availability of concerned specialist over telephone (080-25589225). Beneficiaries are requested to carry original CGHS Plastic/ Self printed card and all medical records. Beneficiaries are required to bring previous prescription and reports of investigations advised during the previous visit.
